If you have an insurance plan that requires a referral or authorization (such as an HMO plan), or for same day appointments, please call our office at 813-771-5500 to schedule to ensure that your insurance requirements are met.
By scheduling an appointment, you acknowledge the following:
- Booking a time slot is a request for an appointment and is subject to confirmation by our office. You may receive a confirmation via email or text message.
- You consent to receive appointment reminders and related communications by email, text message, or phone from Tampa Bay Dermatology and our scheduling partner.
- An appointment does not establish a physician-patient relationship until your visit takes place and you are seen by a provider.
- New patients will be asked to complete intake paperwork and provide insurance information prior to or at the time of their visit.
- Your personal information is handled in accordance with our Privacy Policy and HIPAA Notice of Privacy Practices.
- Tampa Bay Dermatology reserves the right to reschedule or cancel appointments as needed. If you need to cancel or reschedule, please call us at (813) 771-5500 at least 24 hours in advance.
